Privacy notice
Short version: the intake console emails us what you type, we keep clinical records because the law says we must, and we do not sell anything about you to anyone.

What the intake console collects
The form on the contact page asks for a name, a phone number or email address, and optionally your sport, insurance plan name, a target return date and a description of the problem. The phase, visit type and body regions you tag are sent along with it. Nothing on this site is a medical record until you become a patient.
We use those details for exactly one thing before your first visit: replying with slot options and telling you what to bring. If you never book, the request is deleted at the end of the following quarter.
Clinical records are separate
Once you are treated here, your assessment findings, test numbers and session notes live in our clinical record system, not on this website. Those records are handled under HIPAA and New York State record-keeping rules, and they are retained for the period those rules require even if you ask us to delete your web enquiry.
We release clinical information to a surgeon, physician, coach or insurer only with your instruction, except where a law or a court order leaves us no choice.
What this site does not do
- No advertising pixels, no cross-site tracking, no data brokers.
- No cookies set by us for analytics or profiling.
- No sale or rental of contact details, ever.
Web fonts are requested from Google Fonts, which means your browser contacts that service and it sees your IP address as part of an ordinary web request. Our hosting provider keeps standard server logs, including IP address and requested page, for security and troubleshooting.
Children
We treat athletes under 18 in the clinic, but the intake form should be completed by a parent or guardian for anyone under that age. We do not knowingly collect details submitted by a child directly.
Your choices
You can ask what we hold, ask for a correction, or ask us to delete a website enquiry. Email the address above; we answer within a working week and will confirm what was removed and what we had to keep for clinical or billing reasons.
Changes
If we change how the intake console works, this page changes with it and the review date at the top moves. There is no separate newsletter announcing it, so check here if it matters to you.
